These Terms and Conditions apply to all services provided by Striving Care.
By using our services or website, you agree to these terms.
1. Our Services
Striving Care provides NDIS-funded and related support services in accordance with individual service agreements and participant plans.
All services are delivered in line with NDIS Practice Standards and relevant legislation.
2. Service Agreements
Participants will enter into a written Service Agreement outlining:
- Supports provided
- Pricing
- Schedule of services
- Responsibilities
- Cancellation terms
The Service Agreement forms part of these Terms.
3. Fees and Payments
Fees are charged in accordance with:
- NDIS Price Guide
- Service Agreements
- Funding arrangements
Participants are responsible for ensuring sufficient funding is available.
Invoices must be paid in accordance with agreed terms.
4. Cancellations and Changes
Cancellations must be made in accordance with the Service Agreement.
Late cancellations may be charged in line with NDIS guidelines.
We will provide reasonable notice if we need to change or cancel a service.
7. Complaints and Feedback
We encourage feedback and complaints.
Complaints can be made verbally or in writing.
We will investigate and respond in a timely manner.
Participants may contact the NDIS Quality and Safeguards Commission if required.
8. Liability
Striving Care takes reasonable steps to provide safe services.
We are not responsible for loss or damage caused by circumstances beyond our control.
Nothing in these Terms limits rights under Australian Consumer Law.
9. Termination of Services
Either party may terminate services in accordance with the Service Agreement and applicable notice periods.
10. Changes to Terms
These Terms may be updated from time to time.
The current version will be published on our website.
11. Governing Law
These Terms are governed by the laws of New South Wales and Australia.
